If the patient is not low risk by these (treadmill, stress echo, thalium imaging) methods, the options are to:
Intensify medical regimen and reevaluate
Angiography and CABG as indicated
Intensify medical regimen and proceed to surgery with more intense periop monitoring
In general the decision to undergo CABG should be decided on independent of the upcoming surgery
